Hi. In response to question #2 there is a big gap between you 15 deg FW and 5I. In my bag I've fill the space with a 19 deg hybrid and 24 deg hybrid which suits me perfectly. I have a fairly steep swing so a hybrids work for me. It comes down to comfort level but I'd suggest getting 2 clubs to fill the gap - FW or hybrid.

The hosel is adjustable. It ranges from A1 to D4. 

first off d4 is .75 more loft standard lie. i would suggest to bridge the gap I would leave it at standard loft and lie (A1). 21* will bridge the gap between your fairway and your 5 iron.

It refers to the swing weight of the club.

Swing weight is expressed with a letter and number; "D4," for example.

The letters used are A, B, C, D, E, F and G, and the numerals 0, 1, 2, 3, 4, 5, 6, 7, 8 and 9 (G goes up to 10). Each combination of letter and number is known as a "swing weight point," and there are 73 possible swing weight measurements.

D4 is the hosel setting for right-handed clubs that is standard loft/up 0.75 degree loft, so it is 21.75 degree loft. The range on all 910 D/F/Fd/H is +1.5 degrees/-0.75 degrees for both loft/lie.

910H can be 24/21/19/17 and 910F can be 21/19/17/15. Including the adjustments, you have choices. Are you comfortable using a 15 FW off the fairway? If not, do you want a shorter wood? Try the 19/17 910H. Can you get them up ok? The choice between the 5I and the 3W will be personal for feel, and based on needing to fill distance gaps. Spacing the loft is a start, but stil comes down to your distances and your comfort using them.

If you are right handed with a 21* 910h then D4 means 21.75* loft with a lie angle of 58.5*

So if I understand, my 910H 21° D4 will be 21.75° loft (gap=4° with my 5I).

To fill the gap between my FW 15° and this 910H 21° D4, I can choose 910F 17° A4 (18.5°) or 910H 17° A4 (18.5°).

Or I can take only one 910H 21 D1 (20.25°).

Is it easy/possible to change the 910H 21° from D1 to A4 ?

All the 910 D/F/H can be adjusted +/- 1.5/0.75 degrees loft/lie with the settings.  So the 19 can be set 20.5 to 18.25 loft, the 17 will go from 18.5 to 16.25. There is a T-handle tool that will allow you to loosen a screw to move between settings.  The only difficulty is a disqualification if you change the setting during a round.  As a guide, with the fairway having a longer shaft, for the same degree, slower swingers can get better distance (greater head speed) and faster swingers will have better control with the shorter hybrid. 

Find your distance for a 5I and 3W (off the fairway) and then get fitted to get distances inbetween that you need.

What do you think of AP1 4 iron and an 910H 19° to fill this gap ?

I actually have a 26 degree AP1, 24 910H set to 24, a 21 910F, and a 17 910F with another 12 degree driver. I had these from other vendors last year, and the 5I and 4H were definitely different. But the AP1 and working over the winter added 15 to the 5I. Swing speed in low to mid 80's for the driver. Lou G is another believer in fairways, but he is almost as old as me. I just added the 910H and while I know not to expect the distance to be average, I tagged it 190 yards. I only expected 165. Fortunately, I was only trying to lay up, not get within the 10 feet to the pin.

If I could swing 106 mph, and the 4I was as easy to hit as the 5I (last year I also used a 5H because I couldn't hit the wrong 5I consistently), I would carry the 4I and a 19 910H that you could adjust between 4I and the 3W. And you might find the 19H to be long enough to save the 3W for off a tee and very special conditions on a par 5.
